Information and Referral Services:

Intake workers answer phone calls and meet with the general public to provide information regarding any and /or all services available to address identified needs. Referrals may be made to outside agencies, if appropriate. Individuals with questions about community resources or needing agency services may contact an intake worker by telephone or in person.
The intake social worker will provide information regarding any/all services available to address identified needs. Referrals may be made to outside agencies, if appropriate. Individuals needing agency services will meet with our intake social worker to complete an application for services.  All Child Protection reports are received by the intake social worker.

The social worker is also available to explain the reporting statutes, answer any questions regarding agency practices and services, and discuss other resources and strategies for a family to utilize.

Adult Mental Health Case Management:

One agency social worker provides a range of case management and supportive services to individuals and families experiencing problems or difficulties related to mental illness. Services that are available to support individuals and families include case management services, pre-petition screening inpatient admission, civil commitment proceedings, discharge planning, placement, information and referral.
Case management functions include providing referrals to the community mental health center, community outreach programs, day treatment groups, local social club/ vocational programs, residential treatment facilities, board and lodging facilities, money for housing support and assistance from local food shelves.

Children’s Mental Health Services:

Case Management services are available to families with children experiencing severe emotional disturbances. This service helps families identify and coordinate mental health services for children and their families. Services are provided in collaboration with community resources i.e. schools, day treatment facilities and mental health service providers.
Our services are part of a continuum of children’s mental health services – providing education and prevention, early identification and intervention, emergency services, outpatient services, family community support, treatment screening, case management, therapeutic support of foster care, and professional home-based family treatment services – to families with children experiencing severe emotional disturbance.
Social work staff works closely with other agency staff contracted case management practitioners in providing services.
